Eyelid Lift (Blepharoplasty)

The eyelid lift or blepharoplasty is a procedure designed to remove excess fat and skin from the upper and lower eyelids. It improves droopy eyelids and puffy bags below the eyes, features that may make you look older and more tired than you feel. It may be combined with an eyebrow lift to correct drooping eyebrows.

The incision for the upper eyelid is usually made along the eyelid crease. The Incision for the lower eyelid may be made inside the lower eyelid, avoiding external scars (transconjunctival) or externally just below the eyelashes.  Excess skin along the outside of the lower eyelid region may be "tightened" with a chemical peel or ablative laser.*

Forehead Lift

Forehead lifts are usually recommended when a patient comes in complaining of baggy upper eyelids. When the skin of the eyebrows descend, they push the upper eyelid skin down and create bagginess. It is often when a patient realizes that they have baggy upper lids that the surgeon identifies as part of the problem a droopy eyebrow; in this circumstance, the brows are elevated to help improve the results of an upper lid blepharoplasty.

In some instances a forehead lift will be recommended to improve some wrinkled skin and in many instances can help reduce the appearance of a high hair line or a high forehead. There are a number of different approaches for a Forehead Lift, including the direct lift, the mid-forehead pre~tracheal (at or in front of the hair line), coronal (behind and in the back of the hair line) or an endoscopic (minimal incision) approach.*

Face/Neck Lift

Skin of the face and neck lose elasticity as one ages. Underlying muscles also lose tone. This is magnified through sun exposure, weight gain or loss, gravity and stress. These factors may act together and accentuate the appearance of wrinkles, creating a tired look.

A facellft-necklift helps improve the most visible signs of aging through removal and redistributing excess fatty deposits, tightening underlying muscles and removing sagging skin. The standard facelift addresses the lower 1/3 of the face and the upper neck.

Variants include the mini and extended facelift-necklift procedures. Procedure choice depends on your needs arid must be discussed with your surgeon. Surgeons have recognized the three dimensional nature of the facial structures and often complemented this procedure with others to restore volume and address cosmetic issues throughout the facial-neck structures.*

Rhinoplasty

Rhinoplasty (nose) is an operation to improve the shape and size of the nose so that it is in harmony with the rest of the face. Rhinoplasty is essentially a surgical sculpting of the nose and is accomplished by careful contouring of the bone and cartilage giving the nose its shape. At the same time deformities that cause breathing problems and some sinus disorders can be corrected. Most incisions (seamless rhinoplasty) are placed inslde the nose where they are least visible.

Rhinoplasty is performed under sedation with local anesthesia or general anesthesia.  The surgery takes approximately one and one-half to two hours. The use of "seamless" nose surgery allows for no visible external scars and with this type of seamless nose surgery discomfort is minimized because no plates, tubes or painful packaging are used.

Rhinoplasty is often an enhancement to facial rejuvenation, particularly in the correction of the aging nose (drooping of the tip).*

Otoplasty (Ear Reshaping)

Otoplasty (ear reshaping) is designed to correct protruding or disfigured ears in children and adults. Shape, position or proportion of the ear may be corrected. The goal is creation of a natural shape, improving balance and proportion to the ears and face.

Numerous surgical procedures are used to correct ear protrusion. The selection of procedures is based on the surgeon's clinical experience and the underlying cause of the protrusion. Some patient's benefit from placing simple sutures in the ear cartilage to reposition the ears closer to the skull, while other patients need cartilage removal.   Often these procedures are performed in combination.

With children it is suggested to perform otoplasty before the child begins school, to avoid peer teasing and ridicule, which may adversely affects a child's self-image.*

Genioplasty

Many- people have a chin that is just too small for their face. When one has a lack of chin projection, the face is weakened. It can also make the nose look longer than it really is. For this reason, many patients will seek surgery to improve the appearance of the chin. This is accomplished by an osteotomy or by inserting a small synthetic implant over the natural bone. The operation can also greatly enhance the results of a face lift, since loss of chin projection may occur with aging. Chin augmentation is also commonly combined with facial liposuction, where fat is removed from under the chin and from the neck.

A small incision is made either underneath the chin or .inside the mouth.  A sterile implant, similar to the consistency of a normal chin, is placed in front of the bone to increase the projection of the chin. The implant is secured and the incision is sutured closed.  Alternatively, a bony osteotomy may be performed and the chin and soft tissues are advanced to create facial balance and a natural, improved appearance.  Some swelling and bruising can be expected and usually subsides within the first two weeks. A tape of elastic dressing may be recommended for a period of time. Mild discomfort is usually easily controlled with medications prescribed by your surgeon.

After healing, the skin drapes smoothly over the implants. The implants themselves conform to the bone and are usually undetectable. The added projection of the chin results in significant improvement which is especially apparent in profile.*

Chemical Peels

A chemical peel is a treatment that chemically exfoliates the skin's top layers to reveal the softer and healthier skin below.  Chemical peels are used to treat a variety of skin conditions. This can include environmentally damaged, hyper-pigmented, acne-prone, and even sensitive skin types. In manytases, wrinkles caused by sun damage, aging, and heredity can be reduced and even eliminated through the use of a series of chemical peels.  Irregular skin pigmentation, sun-damaged skin, and even precancerous keratosis may improve as a result of chemical peels. Chemical peels can increase the skin's tolerance to external factors and control acne. The strength of the chemical peel is determined by the patient's individual skin type and the degree of damage to the skin. 

This procedure is not effective in the treatment of more severe wrinkles, sagging, and budging which often require a surgical procedure such as a facelift or liposculpture.*

Dermabrasion

Dermabrasion is a surgical procedure whereby the skin's surface irregularities are diminished by abrading or "sanding" the skin with special diamond rotary instruments. Your surgeon carefully reduces the skin surface to a specific depth and after a period of healing the skin is smoother with fewer irregularities.

Often, more than one technique is used to give the best improvement in the appearance of facial scars and wrinkles.  For example, frequently a dermabrasion will be performed several weeks after scar revision. Sometimes, dermabrasion and chemical peel are performed together or in sequence to treat deeper wrinkles, such as those occurring around the lips.

Most of these procedures are performed using a light intravenous sedative and local anesthesia. Sometimes a surgeon will apply cold packs to the area before dermabraslon. Depending on the amount of area to be treated, surgery usually lasts between 30 to 90 minutes. You should experience little, if any, discomfort.

In general, during the first several days after surgery the skin is somewhat tender and swelling can be expected, most prominently around the eyes. Usually after several days the treated areas appear as if you have had a bad sunburn. In most cases you are able to camouflage the areas adequately with makeup after about one week. It is riot at all unusual to have some persistent redness, sometimes lasting several' months. Sometimes, several weeks after surgery small white dots, resembling acne, may appear. These disappear in a short period of time. Occasional areas of darkened skin color may appear. This is usually temporary and may be treated with prescription facial creams. Sometimes the treated areas will appear lighter than untreated areas. While this is sometimes permanent, it is usually quite subtle.*

Laser Resurfacing

Laser Resurfacing Surgery is performed with a "super-pulsed" carbon dioxide (C02) laser to minimize wrinkles and lines on the face. Laser surgery involves a specialized beam of light energy, which is absorbed by the skin and surrounding tissues and minimizes lines or makes precise surgical incisions.  In laser resurfacing, lines on the face can be significantly improved. This procedure is performed under sedation with local anesthesia. Redness may persist for several weeks and make-up can usually be applied in seven to ten days.*

Botox

Botox injections are used to weaken the fine muscles around the eyes and forehead that are responsible for the frown lines and deep wrinkles that occur in these areas. By selectively weakening the proper muscles, the surgeon can eliminate the wrinkles without disturbing normal function.

Treatment consists of a number of tiny Injections made through a very fine needle directly into the muscle. Effects of the injections begin to appear within a few days. The results generally last around four to six months, after which time, the injections must be repeated.*

Injectible Fillers

The use of injectable facial fillers is one of the fastest growing areas in the field of cosmetic surgery. Many advances in have been made in the last 5 years that have led to safer, more predictable and longer lasting fillers .

As one ages, the face loses volume. This creates more prominent wrinkles and skin folds while features become less distinct. Dynamic wrinkles caused by facial movement such as smiling and frowning appear. Injectables were designed to subtly reduce these signs of aging.

Injectables include two basic classes: fillers, to restore lost volume, and botulinum toxin injections, to lessen the appearance of wrinkles with smiling, frowning, concentrating and looking surprised. Most fillers work by filling the lip or wrinkle with volume and are gradually digested by the body. While botulinum toxin injections work by weaken the underlying muscles to decrease their overlying dynamic movement.

FDA approved injectable fillers include Collagen, Hyaluronic Acid Filters, Filters with microscopic beads and Poly Lactic Acid.

For those wishing to avoid the injection of any foreign material, lipotransfer, the harvesting and reinjection of your own fat, may be an option.*

Breast Augmentation

Breast enlargement, known as augmentation, or mammoplasty, can enhance the body contour of a woman who is unhappy with her breast size. This procedure may also be used to improve the loss of breast volume following pregnancy and nursing. Additionally, the operation may help balance breast asymmetries. Saline filled or the newly FDA approved silicone gel Implants may be surgically Implanted by three standard routes with placement either above or below the chest muscle. The standard routes are periareolar (around the areola), inframammary (lower breast folds) and transaxillary (arm pits). Also, saline implants may be placed transumbillcally (via the navel).

The implant is composed of an outer flexible, silicone shell and filled with either saline or silicone gel. The outer surface may be smooth or textured. Implants also have various sizes, profiles, and shapes to meet the individual of each woman.

While breast augmentation will enlarge the breasts, the surgery will not after underlying basic defects in breast shape and form. Major asymmetries may be improved but will not be completely corrected with breast enlargement alone. A noticeable difference in the size, shape or orientation of the two breasts is considered normal and is actually the rule. If breast size and/or nipple position asymmetries are severe, additional procedures to further improve symmetry may be necessary.

Long term experience with breast enhancement surgery has demonstrated that this operation has a high patient satisfaction rate.*

Breast Lift

As women age, the breasts lose shape and firmness. The skin gradually loses its elasticity and no longer holds the breasts in a youthful, upright position. Pregnancy, breast feeding, weight gain and loss also add to the tissue burden of loss of skin elasticity. Women who have sagging breasts (ptosis) may benefit from breast lift (mastopexy).  If there are also problems of excess volume or size of the breasts, a reduction may also be done at the same time. Volume loss may be corrected by breast augmentation at the same time.  Asymmetries may also be improved.

Excess skin is removed, the nipple areolar (dark skin around the nipple} complex is lifted and the breasts are re-contoured to create a more youthful appearance with firmer breasts. Depending on preoperative size, appearance and asymetries, the nipple areolar complexes may also be reduced. There are incisions made directly on the breasts which may extend around the nipple areolar complex, into the front of the breast and into the breast fold, depending on the degree of droop, asymmetry and volume considerations.*

Breast Reduction

Breast reduction is designed to reduce the size of a woman's breasts. It may also improve the overall appearance or shape of the breast and help balance breast asymmetries. It helps in treatment of physical complaints such as neck, shoulder and back pain experienced by many women with large breasts.

Skin, along with breast tissue and fat are removed to create smaller, shapelier breasts. An added benefit where ptosis (droop) exists is a simultaneous uplift of the nipple areolar complex to a more normal position. The areolae (darker skin which surrounds the nipple) may also be reduced in size and made more symmetric.*

Liposuction - Liposculpture

Liposuction, commonly known as liposculpture, involves removing unwanted subcutaneous fat while simultaneously re-contouring to create a more aesthetically appealing body.

While not a treatment for obesity, liposuction-liposculpture selectively removes persistent and unwanted subcutaneous fat.

Bothersome fatty deposits are found in areas that are resistant to. Diet and exercise, such as the face, neck, breast, abdomen, upper arm, hips, thighs, knees,' and ankles. Liposuction-liposculpture can be performed on multiple areas at one time - your surgeon will follow accepted guidelines regarding safe volumes which may be removed in one session.

While goals include size improvement, this procedure is not for weight toss. Final results may not be evident for 6 months following your procedure. During this time your body balances fluids and the skin and remodels underlying tissues. Final results also depend on proper nutrition, exercise and decreasing your caloric intake.

Small incisions are made in hair bearing areas or natural skin folds. A special tumescent solution is next placed within the fatty areas to be removed. This solution provides anesthesia to the areas, while limiting the usual blood loss. Fat is then carefully removed from the areas of concern to the patient using a surgical instrument called a cannula, which Is attached with tubing to a suction machine.

The time for completion of the procedure depends upon the total amount of fatty tissue removed. At the end of the procedure, the skin openings may be sutured or left open to heal, depending on the size of the openings. The patient may be placed in a compression garment that is worn for certain amount of time afterwards.

While recovery is easy, you should expect some initial drainage along with swelling, bruising and discomfort. Your compression garment will minimize the swelling, bruising, discomfort and assist in the retraction of any loose skin.

While some results are noted immediately, your final shape and form will continue to improve as your swelling subsides. You will be encouraged to be up and about the night of your surgery and progressively increase your activities. Usually light work duties may be resumed within three to five days and vigorous exercise within two to four weeks.*

Abdominoplasty (Tummy Tuck)

Abdominoplasty (tummy tuck) is an operation that is designed to reshape the abdomen. Often, through weight gain and loss, pregnancies and aging, the skin stretches beyond its elastic limit or ability to return to normal shape. An abdominoplasty removes excess skin and fat while the underlying muscles are tightened.

The actual placement of incisions and design of the abdominal incisions vary depending on the looseness of the condition underlying tissues, the amount of fat to be removed, the of the overlying skin and prior scars.

For those women where the sagging skin is mainly in the lower abdomen the mini-abdominoplasty is an additional option. This procedure is not as extensive due to a smaller incision that is easily hidden under a two piece bathing suit.  This procedure offers a more rapid recovery and may be done on an outpatient basis.*

Sclerotherapy

The standard treatment for spider and varicose veins is Sclerotherapy, which involves injecting a chemical (usually salt-water, or saline) solution into the blood vessel, which then collapses and is absorbed by the body.  Alternative treatments include laser or intense pulsed light therapy, electrodessication, surgical ligation and stripping (requiring hospital stay) and ambulatory phlebectomy.*
